Physical Health & Wellbeing Screen

The MyPulse health and wellbeing screens are a series of questions about behaviours, actions and feelings - they are completely confidential; take between 5-10 minutes each to complete and you can even do them on your mobile device – anywhere, anytime.

On completion of the screen, if you present with moderate or high-risk factors you may be contacted by one of our qualified MyPulse Coaches and offered free, confidential and tailored health coaching to help maintain and improve your wellbeing. If your answers demonstrate positive health and wellbeing you will receive a completion certificate via email.

Health screens explained

Answer all questions to the best of your ability - there are no right or wrong answers, and no judgement. This screen is designed to increase your awareness of your lifestyle and risk factors on your physical health and wellbeing. All fields marked with an * are mandatory.

All your responses are held in the strictest of confidence and will not be shared with your employer or any party unless we have your written permission. For further information on how your data is used and protected visit our Privacy Statement. Your work email is a requirement to ensure your access and your preferred email, phone number and name will be used solely for the purposes of coaching should it be recommended.
